How to Get Help

Financial Assistance Eligibility Requirements

LEAVEN has just one eligibility requirement - you must be at least 18 and residing in Outagamie, Calumet, or northern Winnebago Counties. We believe in providing minimal-barrier access to resources for those in need.

Required Documentation

When you meet with the LEAVEN team, please bring proof of identification. You should also bring any papers that support your request for assistance, such as:

  • Car repair estimates

  • Disconnection notices

  • Eviction notices

  • Childcare invoices

  • Lease agreement

  • Mortgage statements

  • Rent statements

  • Utility bills

  • Step 1: Contact LEAVEN

Contact us via phone, email, or our contact form to schedule an appointment.

  • Step 2: Meet with LEAVEN

Meet with a member of our team at our office located at 1475 Opportunity Way, Menasha, WI 54952 to discuss your emergency needs. This appointment is anticipated to take about 1 hour. When you come to LEAVEN, please bring some proof of identification and the required documentation.

  • Step 3: Connection to Resources

Recognizing that households seek assistance during emergencies, we strive to make connections shortly after the appointment; however, timing may vary case by case and depends on the household’s follow-up related to their request.
